The goal is to promote patient's well-being by providing high quality nursing care.License Practical Nurse Job DescriptionAdmit referrals for Personal/Respite care, Create and update plan of care as necessary, and document and communicate actions to maintain continuity of care. Comply with procedures, regulations, rules in maintaining medical records. Perform assessments on clients to assess their conditions and home environments. Teach clients and their families about health care in the home. Data entry submission of documents electronically as required for various authorizations.Education,Experience, and Licensing RequirementsLicense Practical Nurse, Active Applicable Virginia License,American Heart Association issued BLS and ACLS. Please have at least 2 years as and LPN and 1 year in the home health industry.Please have dependable transportation. Serious inquires only. Pay Base on experience.
